Resumen
Objetivo: Describir los ingresos hospitalarios ocasionados por la varicela y sus complicaciones en nuestro centro, y compararlos con otros estudios publicados.
Pacientes y método: Estudio retrospectivo de las historias clínicas de los pacientes menores de 14 años ingresados por varicela o alguna de sus complicaciones entre los años 1990 y 2000. Se excluyen neonatos. Se realiza un análisis estadístico descriptivo de los datos.
Resultados: Recogemos 65 niños ingresados por varicela y sus complicaciones (3,9% de los casos de varicela que acudieron a la Unidad de Urgencias Pediátricas). La media de edad fue de 4,4 ± 2,7 años (72,3% de 5 años o menos). La mayoría de los pacientes (58,5%) eran niños previamente sanos. La mediana de la estancia hospitalaria fue de 5 días (rango 1-42 días), precisando 5 niños ingreso en UCIP. Las complicaciones más frecuentes fueron las neurológicas (32,8%), seguidas de la sobreinfección de la piel o tejido celular subcutáneo (26,2%). Se presentaron también complicaciones respiratorias (14,8%) -especialmente neumonías-, gastrointestinales (8,2%), hematológicas (6,6%) y osteoarticulares (4,9%). Se detectó infección invasiva por estreptococo del grupo A en dos pacientes con celulitis. No se encontraron casos de shock tóxico estreptocócico, fascitis necrotizante, síndrome de Reye o fallecimiento.
Conclusiones: Pese a su benignidad habitual, la varicela puede producir complicaciones graves, cuya distribución varía por condicionantes geográficos y socioeconómicos, y ocasiona un considerable gasto sanitario. La existencia de una vacuna contra la varicela, especialmente eficaz para prevenir las formas graves, debe hacer que se valore su introducción en nuestro calendario vacunal.
